There are many ways that a mutation can occur, any "mutagen" (a "substance" the can potentially mutate your DNA sequence) can technically cause changes in your nucleotide sequence.
Meiosis and mitosis of the cells are NOT perfect mechanisms and mutations do occur in the cells without being fixed sometimes.
U.V radiation/X-rays
Some chemicals if working in a lab.
Through actually adding in a new nucleotide sequence (common studies done in yeast).

a mutation can occur if the base on the DNA is removed, replaced, or changed. If a base is removed it can cause a serious mutation. The sequence will be shortened and no protein will be made. If it's replaced the whole sequence will change and produce a different or sometimes no protein. If its changed then it can cause the production of proteins to immediately stop, which can be a problem for the person. If its change with another it can cause another protein to be produced or abruptly stop during protein synthesis.
